DeWayne Lewis Dies in Tragic Crash on 5th Avenue [Phoenix, AZ]

Phoenix, AZ (September 11, 2026) — Phoenix police are investigating a fatal hit-and-run crash that killed 57-year-old DeWayne Lewis near 5th Avenue and Indian School Road.

Officers responded around 6:30 p.m. and found Lewis after a vehicle struck him. Paramedics took Lewis to a hospital, where he later died from his injuries.

Police believe a white SUV hit Lewis and fled before officers arrived. Indian School Road between 3rd and 7th Avenues remained closed for several hours as investigators worked at the scene.
